Preparing for Your Visit
Here you can find patient forms, insurance plans, policies, pre-testing preparation, and hospital affiliations — so you know exactly how we operate and can feel comfortable and confident putting your health in our hands.

Your First Visit
Please arrive about 15 minutes before your appointment so we have time to check you in. Plan on being with us for around an hour for a first visit — longer if testing is scheduled the same day.
What to bring
- Your insurance card and a photo ID
We copy these at check-in. Bring your current card even if nothing has changed. - A complete list of your medications
Include the dose and how often you take each one, plus any vitamins, supplements and over-the-counter medicines. Bringing the bottles themselves is perfectly fine. - Any recent heart records you have
EKGs, stress tests, echocardiograms, catheterization reports or hospital discharge papers from another provider. These save you from repeating tests. - Someone with you, if it helps
You are welcome to bring a family member or friend to help you remember what we discuss and to ask questions. - Your referral, if your plan requires one
Some insurance plans require a referral from your primary care provider before they will cover a cardiology visit. Check with your plan if you are unsure.
